Dual-dose device provides back-up medication for severe allergic reactions

Verus Pharmaceuticals, Inc. today announced the availability of its enhanced Twinject epinephrine auto-injector for anaphylaxis. Twinject is the only auto-injector with two doses of epinephrine in a single, compact device.

"Verus is committed to providing innovative solutions for anaphylaxis patients and their caregivers," said Robert W. Keith, President and Chief Operating Officer of Verus. "The enhanced Twinject is one of several line extensions and next generation products we have in our current anaphylaxis pipeline and we are pleased to bring this easier to train and simpler to use product to market in the U.S."

Anaphylaxis is a severe, life-threatening allergic reaction triggered by exposure to one or more various antigens, including foods, insect stings, drugs, and latex products. Up to 43 million people in the U.S. alone are at risk for anaphylactic episodes, and underlying incidence rates are expected to continue increasing in future years. Up to eight percent of children have food allergies, and sensitivities to peanuts and tree nuts among children have doubled in the past five years.
Reading about skateboarder Andy Kessler’s death from an insect sting, brought back all too vivid memories of my husband’s brush with death. We were visiting my brother, Jimmy, in Birmingham a few summers ago. His home is nestled in the woods in a sparsely-populated, gated golf course community. We were invited to his neighbor’s for a swim, which was almost a mile away by car. As our children swam and the adults chatted, we apparently disturbed a wasp, because he stung my husband, John, in the head. Just a few minutes later John began to feel very strange and he interrupted me holding out his hands and said they were “tingling.” Our hostess headed to the house to look for Benadryl as I began to sense that we may need something more. Here’s where we are lucky – although we never considered it before - our 16 year old daughter has a life-threatening allergy to peanuts and tree nuts. We have known this since she was 15 months old and we never travel anywhere without her emergency medicine called an Epi-pen (an auto injector used to stop a potentially life-threatening reaction). Not only that, but I had read extensively about anaphylaxis (a severe allergic reaction), trying to learn all the signs and symptoms, along with how to act quickly in response.I insisted John get in the car because I wanted to get back to Jimmy’s house where I knew we had Benadryl and an Epi-pen if needed (we didn’t take it to the pool because it was very hot out and excessive heat can compromise the medication).  Unfortunately, I did not communicate to anyone what I was thinking, “this might be anaphylaxis,” and we drove off leaving the other adults watching our children in the pool.It only took a minute or two to realize I had made an irreversible mistake. My husband’s heart was racing and he was getting very anxious. He verbalized the telltale ‘sense of doom’ and was repeatedly telling me “this is not good.” Moments later his head fell back and he was struggling to breath. I drove as fast as I could along the windy road, sped into Jimmy’s driveway, parked the car and raced up the front steps. In the house I grabbed the portable phone and raced to the second floor, praying that I’d find the Epi-pen with my daughter’s suitcase. My mind was also racing… “I don’t know the telephone number to the guard at the gate… I don’t know how to reach Jimmy… why didn’t I bring help?” Luckily the Epi-pen was exactly where I thought it was and as I ran down the stairs my fingers instinctively dialed 911.I was able to maintain composure with an Epi-pen in my hand and an emergency operator on the other end of the phone but the reality was that we were a long way from help and my husband was in shock and barely breathing. He was in the passenger seat of the car and I administered the Epi-pen into his thigh. This was my first time to actually give an Epi-pen shot, but the family practices by injecting oranges with expired Epi-pens (I highly recommend this).I don’t think anyone’s sense of time can be trusted during a crisis like this, so when I complained to the 911 operator that the Epi-pen shot should have worked by now, I don’t know how long it had been. But we always travel with at least two Epi-pens, having read that a second should be used if there was no improvement in 10 minutes. The operator, however, convinced me to wait because “it’s not his prescription and he’s still breathing.” Honestly, at that point I preferred to follow her instructions, but I told her that I would check the expiration date. As I reached for the used Epi-pen on the floor of the car, I accidentally knocked the other one onto the ground. The case opened and it ejected, losing all the medication. I informed the operator, “We don’t have to argue about that anymore.”Then came the rain. The skies opened up as I stood there, watching John for any sign of improvement. And just as I heard the sirens in the distance, there was an ear-splitting clap of thunder that sounded like it was right on top of us and John lifted his hand to hold his head in response. As he slowly came to, the fire engine pulled into the driveway and the nightmare was beginning to end.John received a couple more doses of epinephrine before he reached the hospital. Luckily, the ‘first responders’ in Alabama came equipped with it, but that has not always been the case. Providing first responders with epinephrine has been a state by state process that organizations such as the Food Allergy & Anaphylaxis Network (FAAN) have had to lobby for.We all felt very fortunate when I brought John home from the hospital several hours later. But I was left with nagging thoughts about the day’s events: I should have realized it was anaphylaxis sooner; I should have never driven off with John by myself (what if he had needed CPR? I would not have been able to get him out of the car.); I should have trusted my knowledge to give him the second Epi-pen. But no matter how critical I am of my performance that day, there is no doubt in my mind that the Epi-pen saved my husband’s life and if I had not prepared myself so well for our daughter’s food allergy I would not have been prepared for saving my husband from the deadly insect sting.I share my story because I hope people will learn from our experience, as I learn from others. This year I am a volunteer and co-chair for FAAN’s 2009 Walk for Food Allergy, September 26 at Brook Run Park in Atlanta www.foodallergywalk.org  It’s a wonderful morning where families can come together, have fun, and work toward increasing education, awareness, and finding a cureÂ

Author: MultiVu Added: Wed, 28 Mar 2007 15:03:12 -0800 Duration: 60 Report Stresses Need for Better Patient, Doctor and Food Service Staff Education

(Fairfax, Va., March 28, 2007) /PRNewswire/ — A new report on fatalities from food-induced anaphylaxis, which follows up an earlier study, suggests that little progress has been made in effectively preventing and treating food allergy reactions over the past five years. The findings support, alarmingly, earlier statistics showing that adolescents and young adults are at highest risk for fatalities. The need for more education on the part of the medical community and patients, more attention focused on reading labels and avoiding allergens, and the importance of carrying and using epinephrine are some of the major areas which need to be improved.

The report, which appears in the April 2007 issue of the Journal of Allergy and Clinical Immunology(JACI) looks at 31 individuals who died as a result of their food allergies. This report follows up one from 2001 in which 32 food-induced fatalities were examined.

"These statistics are disturbing," says Anne Muñoz-Furlong, Founder and CEO of the Food Allergy & Anaphylaxis Network (FAAN), "because they show we are not making the progress we hoped to in preventing food allergy-related deaths or in protecting teens, the highest at-risk group for fatal reactions. It's going to take more awareness and action to make a difference."

The foods primarily responsible continue to be peanuts and tree nuts, such as walnuts, almonds and hazelnuts, but also include milk and shrimp. The most common food sources were restaurant or food service items with hidden allergens (46%) and packaged food with undeclared allergens (27%). Desserts, particularly cookies and bakery items, and Chinese food such as egg rolls caused a significant number of the reactions. In 13% of cases, the food items were homemade. In addition, every individual, for whom there was information, had asthma.

Most of the individuals were not carrying their self-injectable epinephrine, and some had never been prescribed this life-saving drug. A June 2006 study entitled "Risk-Taking and Coping Strategies of Food Allergic Adolescents and Young Adults," also published in JACI, showed that adolescents often don't carry their epinephrine self-injectors because they are self-conscious about carrying something that makes them look different from their friends.

"The solution starts with better education of medical professionals at all levels. Physicians need to do a better job of diagnosing patients and providing adequate evaluation. They also need to redouble their efforts to emphasize the importance of allergen avoidance and carrying epinephrine at all times," reports the study co-author Allan Bock, M.D., an allergist in Colorado.
